With the new navigation tabs, your team can quickly move between population management templates and billing templates. There’s less searching and scrolling, making systems much more intuitive to use.
New CCM Care Plan Dashboard Practice Template
The Chronic Care Management 2.0 Enhancement also includes the new CCM Care Plan Dashboard, a practice-level dashboard that helps teams manage chronic condition care plans.
In just a few clicks, the care team can:
Review upcoming due dates, e.g., for procedures, checkups, or tests
Monitor care plan status
Immediately access patients’ charts for follow-ups or documentation
Users can now sort, filter, and search by patient, ID number, status, and other keywords and phrases, making it much easier to quickly access relevant information and ensure no aspect of a plan is missed.
CCM Management Template Enhancements
Effective enrollment of patients into CCM care plans requires accurate documentation and careful tracking. With that in mind, new template enhancements include:
Visibility of patient enrollment dates
The ability to document when a patient declines CCM services
A new field to capture patient consent and the consent date
After enrollment, your team can quickly view key information, including practice, provider, and location details, as well as payer details, including self-pay patient data.
CCM Care Plan Template Enhancements
The CCM Care Plan Template is now used to streamline the documentation of key care plan components.
Clinicians can quickly document:
Health concerns and goals
Necessary Interventions
Patient outcomes
Current care team members
Functional and cognitive assessments
Using these templates to document relevant patient information makes it simpler for the next medical professional to retrieve and quickly understand patient journeys. Telephone Call Template Enhancements
The new enhancements also simplify documenting time spent on CCM services. Clinicians can log CCM time as telephone calls or telephone communication, as well as face-to-face or other methods. Time spent with chronic care patients is automatically logged for effective tracking and to reduce manual input.
Provider Test Action Template Enhancements
The Provider Test Action Template has been enhanced to enable faster reviews of CCM patients and further streamline time logging.
Providers can now:
Review appointment and contact information
Send messages to other team members
Document CCM time in a single location
The Add CCM Time button is available in the Action Items/Log panel, reducing the need to switch between templates.
