What is a Relative Value Unit (RVU)?

Physician payments in the U.S. are typically calculated using a fee-for-service model.  But, it is not as simple as it sounds. A single service doesn't necessarily equal a set amount. Physician payments are often based on relative value units (RVUs). This standardized metric is the foundation of the resource-based relative value scale (RBRVS). RVUs do not directly compare to dollar amounts; they define the value of provided services and procedures. 

This value measure is based on the physician's work, clinical and non-clinical resources, and expertise required to care for patients. RVUs help ensure consistent valuation of medical services across various specialties, geographic locations, and practice settings. Because RVUs are linked to reimbursement, they are vital in the medical billing process. However, they are involved in performance analytics and productivity measurements, too. The RBRVS model uses three types of RVUs to capture the full extent of the time, money, and effort involved in providing patient services.

RVUs vs. CPT Codes – What's the Difference?

While RVUs and CPT (current procedural terminology) codes play roles in medical billing, they serve different functions in healthcare reimbursement.

CPT Codes:

  • Developed and maintained by the American Medical Association (AMA)

  • Alphanumeric codes that identify specific medical procedures and services

  • Used by providers to report services to payers for reimbursement

  • Updated annually to reflect changes in medical practice and technology

  • Directly entered on claims forms during the billing process

RVUs:

  • Established by CMS with input from the AMA's Specialty Society Relative Value Scale Update Committee (RUC)

  • Numerical values that quantify the relative value of services

  • Used by payers to determine reimbursement amounts

  • Adjusted annually based on changes in medical practice and resource costs

  • Not directly entered on claims forms but associated with each CPT code

The Relationship between RVUs and CPT Codes

Here is how RVUs and CPT codes relate to one another, and

  • A CPT code has corresponding RVU values assigned to it. So, the CPT codes describe medical procedures and services. In contrast, RVUs are a numerical system that assigns value to those procedures and services. Know that providers do not assign RVUs directly when coding and billing. Instead, they assign the appropriate CPT code, and the corresponding RVU values are applied behind the scenes during reimbursement calculations.

  • When a provider bills a specific CPT code, the associated RVUs determine the service's worth relative to other services. For example, a complex surgical procedure (represented by its CPT code) will have significantly higher RVU values than a routine office visit. These higher values reflect the degree of physician work, practice expenses, and malpractice risk.
